Dear sir / madam
I am writing this to inform you that i have been suffering from food poisoning since last night. the doctor has advised me to take rest for atleast two day with some medications. hence i request you to approve sick leave for two days from staring date to ending date. in case of emergency i will be available at my mobile number.
