Dear Sir,
Re: Leave Request
I am writing to request your kind consideration in approving my intended leave on October 25, 2010. I apologize for the short notice given, but the specific date is to go for a specialist check up at the General Hospital.
Attached is a copy of my attending doctor’s recommendation for a brain scan at the General Hospital as I have been experiencing terrible bouts of a headache for the past three months. Regular painkillers are ineffective and the doctor, who is also a company panel doctor, has strongly advised for a brain scan to identify the source of pain.
I thank you in advance for your kind consideration on my request. I do hope that I can continue to serve this esteemed company for a long period of good health and full concentration.

Since February 1 and 2, 2011 are the weekend; I shall be back to work on Monday, February 3, 2011. It is just one day leave that I am applying for. My husband and I will be on going on a short trip at that weekend.
I have briefed my assistant, Ms. Martha Manor, of all outstanding issues that need to be dealt with in my absence. But I believe all are in good order although it is the month end; all monthly reports would have been submitted by the 25th of the month.

I was all ready to come to work, but an urgent phone call from my mom in Phoenix stopped me at the door. She informed me of my father’s accident, and he was to be operated on that morning.
Hence, I had to rearrange my plans and drive back to my hometown immediately. My father was successfully operated but need convalescence care which my mom could not handle alone. I had to arrange the necessary home medical care for my father on her behalf. Once the arrangements were worked out, I headed home and reported to work yesterday.

Maternity Leave letter
A Maternity letter is submitted by an expectant mom to her employer
for the days of rest when her baby is born. Sometimes the company allows
the maternity leave to be effective slightly earlier than the actual
birth date for the expectant mom to rest at home. The number of days in a
maternity leave is dependent on the labor law or company regulations,
whichever is more favorable to the mother.

RE: School Leave
I, Emily Gulley, parent of Wilma Gulley, a grade 3 student at Holden High School, wish to inform you that my daughter will be absent from school from January 15-20, 2011.
The whole family will be away that week for an overseas trip. It is a last minute trip that has to be made due to some unforeseen circumstances beyond our control. As such, I apologize that my daughter, Wilma Gulley, will not be present in school for those days.
I would appreciate it if you could inform Wilma’s teachers of her absence and to assist her in the necessary school work before she goes off. She will work on her assignments and projects where possible if necessary.
Thank you for your kind understanding and assistance.
Respectfully yours,
Emily Gulley
