PF request letter - Provident Fund

Sub : Requisition to create PF account

I would like to bring to your notice that I am being working here as Web Designer since 15th Jan 2010. This is to let you know that Provident Fund account has not yet been created for me.

It would be great if you arrange to create the PF account. Hope to hear from you soon.
